TEACHER SEMINAR APPLICATION

• Thank you for using the 2008 Online Application. The form must be completed entirely. Incomplete forms will delay registration.
• Please submit a separate form for each person registering and mail a printed version to our office with your signature.
• Your credit card will be charged the full amount of tuition and fees.
• Cancellations must be made in writing to info@sagaworkshop.org no later than October 15. No refunds will be issued after October 15.
• Expect a response via email. Please give us a valid email address that you use regularly. Do not accidentally delete our response as Spam—the subject line is “SAGa Registration confirmation”.
• Allow one week for your registration to be processed. If you do not hear from us by then, please contact us immediately.
